Same-Day vs. Delayed Dental Implants: Timing Your Treatment

If you have a tooth that is badly cracked or has deep decay, removing it is often the first step to getting your health back. If you are planning to replace that tooth with a dental implant, one of the first questions we will discuss is the timing: should we place the implant immediately during the extraction visit, or should we delay it to let the area heal first?

Understanding the Clinical Protocols

In implant dentistry, we look at the health of your bone and gum tissues to choose between two main paths:

• Immediate Implant Placement: During the same appointment where we extract the damaged tooth, we place the titanium implant post directly into the fresh socket. This is often called "same-day implants."
• Delayed (Conventional) Placement: We perform the extraction, place a bone graft to keep the socket stable, and allow the area to heal for 3 to 6 months before placing the implant.

What the Clinical Research Says

A systematic review of implant timing published in the Cochrane Database of Systematic Reviews compared patient outcomes across these protocols. The review confirmed that both methods are highly successful when patients are selected carefully.

Immediate implants are very reliable as long as there is enough healthy bone surrounding the socket to lock the implant tightly in place during surgery (this is called achieving primary stability). However, if the tooth was lost due to a severe abscess or deep gum infection, we prefer the delayed approach. Letting the infection heal first ensures the implant has a clean, sterile environment to bond with your bone.
